Employment Eligibility Verification

Federal law requires that every employer* who recruits, refers for a fee, or hires an individual for employment in the U.S. must complete Form I-9, Employment Eligibility Verification. Form I-9 will verify your identity and employment authorization.​

Directions:

  • Click on the link for Form !-9​​

    • Fill out Page 1 - Section 1 of the form

    • You will also need documents from the Documentation Lists below

      • Most common:​

        • Driver's License (List B) and Social Security Card (List C); or​,

        • Passport (List A)

    • email copies of the Form I-9 and support documents to HR​

How To Submit

  • On your phone:

    • Open form and fill out

    • Take a screenshot of completed form

    • use edit app to sign form and save

    • take a picture(s) of the document(s) from list A or list B and C

    • email form and pictures of documents

  • On your computer:

    • ​print out form

    • sign the bottom of the form

    • scan a copy of completed form and document(s) from list A or list B and C

    • email form and documents
